NCDHD Plans Free H1N1 Community Clinics in Elgin

North Central District Health Department (NCDHD) will hold two free H1N1 flu vaccine Community Clinics with the first beginning at the Elgin Public High School and will operate from 8:00 a.m. until 9:30 a.m. and then at Pope John High School from 10:00 a.m. until 11:30 a.m. in Elgin on Friday, December 18, 2009.   

The Health Department will offer the H1N1 flu vaccine free of charge to people in the community of Elgin, Nebraska. 

Only the people identified in the CDC’s designated target groups will be served.  Those groups are:  1) pregnant women;  2) children from six months through 24 years of age;  3) people who live with or care for children younger than six month of age;  4) people between the ages of 25 and 64 years with chronic health conditions that put them in danger of complications, and;  5) healthcare and emergency services personnel.

People can reduce the time it takes to get through the line by filling out their vaccination consent form in advance.  

These forms can be found on the North Central District Health Department’s website at:  www.ncdhd.ne.gov. Click on the “H1N1 Flu Vaccinations Consent Form” icon located on the home page.  Print out this form, complete the required information and bring it with you to the clinic on Friday, December 18th. For those without computer access, forms are available at the clinic and can be completed on-site.
